Supporting San Diego Through Palliative and Hospice Care

Understanding the role of palliative and hospice care is essential for families navigating the complexities of serious illness. Cheer Home Care recently hosted LightBridge Hospice and Palliative Care for a Lunch and Learn event, offering education and resources to healthcare professionals, caregivers, and families in the San Diego community.

Greg Hahn: A Leader in Palliative Care

Greg Hahn, a respected figure in the healthcare industry, has dedicated more than 20 years to LightBridge Hospice and Palliative Care. With a background in nursing and a passion for patient care, Hahn is recognized for his pioneering work in the field! During the Lunch and Learn, he shared his expertise on palliative care’s critical role in managing symptoms, improving quality of life, and ensuring continuity of care.

What Is Palliative Care and Who Can Benefit?

Palliative care is specialized medical care that focuses on relieving the symptoms and stress of a serious illness. Unlike hospice care, which is designed for those with a prognosis of six months or less, palliative care can be provided at any stage of an illness and alongside curative treatments. Anyone can make a referral, whether it’s a family member, caregiver, or healthcare provider.

LightBridge has been a staple in San Diego’s healthcare landscape, being one of the region’s first palliative care providers. It is one of only two palliative care programs in San Diego covered under Medicare Part B, making its services more accessible to those in need.

The LightBridge Difference: Experience You Can Trust

LightBridge is an award-winning hospice provider known for its compassionate, patient-centered approach. Their team works alongside a patient’s existing medical professionals to ensure smooth care coordination. When a palliative care assessment is conducted, the findings are shared with the patient’s primary care physician, keeping them informed and involved in the treatment plan.

Palliative care also can serve as an excellent bridge for patients transitioning from hospice, ensuring continued support and symptom management. This model of care is ideal for those experiencing a change in condition, managing complex symptoms, or aiming to prevent hospital readmissions.
